Imdb and wikipedia both state that the screening of "Plane Crazy" in Hollywood on 15 May 1928 was the first appearance of Mickey Mouse on the screen, a date preceding that offered by the Disney archives.
A.Q. 6/1964 quotes data department for the time of the first screening. Bryan Aspland confirmed with the official Disney archives that Mickey and Minnie were born at the Colony theater in New York, appearing in "Steamboat Willie," given in AFA DX No.106, taken from the Data Section of AJ's summer edition.
